Medical Center!Job Summary and QualificationsAs a Registered Nurse
for Admission and Discharge, the top priority is delivering
exceptional patient-centered care. They are dedicated to ensuring a
seamless and efficient transition for patients coming in and out of
acute care hospital settings. Specialized assessment skills help
ensure that patients are placed in the appropriate care areas and
discharged. We offer education and post-hospitalization care as
necessary. Their main focus is advocating for patients, families,
and caregivers while embodying the organization's vision, mission,
and values. The goal is to give an unmatched patient experience and
attain optimal clinical outcomes.Responsible for facilitating
effective throughput at Wesley Medical Center. Responsible for
identifying patients fit for the discharge lounge, coordinating
with a multi-disciplinary team, retrieving patients from hospital
unit, and providing discharge education to the patient.
Collaborates with unit leaders, physicians, operations staff and
other professional disciplines to ensure patients are ready for
discharge and to ensure effective patient care delivery and
achievement of desired patient outcomes. Responsible for directing
and coordinating all nursing care for the department -specific
patient population in accordance with the standards of nursing care
and practice as defined by Wesley Medical Center and the Kansas
Nurse Practice Act. Supports the organization vision and mission.
Utilizes knowledge of patient's age and cultural diversity in the
provision of patient care. Contributes to the provision of quality
nursing care through performance improvement techniques that
demonstrate positive outcomes in patient care.What will you do in
this role:
- Obtains a nursing assessment history, and ensures proper
inpatient care unit admission. Takes appropriate action when
needed, such as communicating special circumstances to physicians
and/or care team or reporting indications of abuse or neglect.
- Completes and facilitates the completion of required
documentation, such as admission assessments, risk assessments,
medication reconciliation, discharge paperwork, and post-hospital
documentation.
- Teaches patients/families/caregivers about a patient's medical
condition, patient status, treatment plan, medications and possible
side effects, and follow-up measures. Translates complex medical
terminology to ensure complete knowledge.
- Completes extra discharge documentation, education, and
discharge care for special patient populations, to ensure
compliance with core measures for discharge.
- Ensures patients /families/caregivers receive copies of
discharge paperwork and any other documentation needed, such as
referrals for ongoing care, educational materials, etc.
- Coordinates with House Supervisors/Nursing Administration to
prioritize workload and identify greatest need areas to ensure
smooth patient transitions.
- Demonstrates a time-conscious awareness and ability to
prioritize. Distinguishes between low and high-priority
activities.What qualifications you will need:
- Current Licensure in the State of Kansas as a Registered Nurse
(RN) or Current Multi-State Licensure as a Registered Nurse.
- -Current Basic Life Support (BLS) verification course through
AHA or ARC
- Graduate of an accredited school of professional nursing
- Ability to utilize the nursing process in the provision of
nursing care including, but not limited to, administering
treatments and medications, interpreting diagnostic tests,
formulating a plan of care, collaborating with other health care
providers and providing education to patients and/or significant
others
- Demonstrated ability to communicate effectively
- Possess personal computer skills
- Ability to multi-task, delegate and prioritize
- Possess problem solving and critical thinking skills
- Ability to collaborate with other health care providers
- Ability to educate patients and/or significant othersPreferred
