• Certification Details
• BLS Certification - Must be AHA.
• Nursys - Nursing only Michigan hospitals need their licensure verified through LARA, not NURSYs
• License/Certificate Verification - Current license and primary source verification within 30 days prior to start of assignment
• MMR Vaccines or Positive IGG Titer
• Fit Test - Required for all disciplines, Annual
• Hepatitis B Documentation of vaccination series completion or declination to vaccine
• Influenza Vaccine - Required seasonally
• Positive PPD- Negative Chest X-Ray and Annual TB Questionnaire
• Health Statement - Statement of good health or completed physical exam prior to first assignment, updated annually
• 2 Step PPD OR IGRA - Proof of Chest X-Ray within last 1 year to be accompanied by TB questionnaire if positive TB record
• Varicella - Proof of immunity or vaccination required
• Tdap - Valid 10 years from the administered date

• Ascension Skills Checklist - Required annually in specialty
• Ascension Color Vision - Completed prior to start and updated every 3 years
