Job Description Summary:
Must be AHA BLS certified, current license, and primary source verification within 30 days prior to start of assignment. Ascension Color Vision needs to be completed prior to start and updated every 3 years.

  • --- Statement of good health or completed physical exam prior to first assignment, updated annually thereafter

  • --- Evidence of immunity, vaccination or titer results for Varicella
    
  • --- Evidence of immunity, vaccination or titer results for MMR
    
  • --- Tdap vaccination valid for 10 years
    
  • --- 12 Panel Drug Screen within 30 days prior to start and updated annually
    
  • --- Ascension Education Verification with no gaps
    
  • --- Confirmation of Assignment to be uploaded by the agency
    
  • --- Influenza Vaccine Exemption/Declination
